Here's a list of the ones I found freaky:

Dying For Pie- When SpongeBob performed open-heart surgery.
Chocolate With Nuts- The old lady was freaky!
Mermaid Man and Barnacle Boy IV- When Squidward was cut by the scissors!*shudders* Also, when a female fish tore off one of SpongeBob's lungs and when Squidward was sawing one of SpongeBob's veins!
Doing Time- When Mrs. Puff accidentally peeled off the skin of the two police guards.
SpongeBob B.C.- It was scary to see Patchy get eaten by the dinosaur!
Krabby Land- When the truck drove over SpongeBob's tongue!
Nature Pants- When the poison sea urchins were on SpongeBob.
SpongeBob Meets the Strangler- When SpongeBob stepped into the Strangler's eye sockets!
Wet Painters- When Patrick was getting beat up by SpongeBob!
Good Neighbor- When Squidward rested on the rock uncomftorably!

Freakiest episode= Mermaid Man and Barnacle Boy IV!

Patrick has no nose but he has a nostril built right into his face. The reason he can't smell is because there's so much hair blocking it.
