Remember that Ozai approached Azulon and wanted the throne which by all rights belonged to Iroh who had just lost Lu Ten in the war. Azulon was hacked and said to Ozai his punishment would be to know what it was like to lose his firstborn son. So all we see next is that Lady Ursa is frantically saying good-bye to a half-awake young Zuko, reminding?? him that she loves him and for him to never forget who he is. Then she disappears and is suddenly gone and is absent from Ozai's coronation as Fire Lord and Azulon is dead. Lots of questions with very little answers.
