I will use the episodes as examples.
Blind Bandit:
It showed her in the tournament where she was sensing the movements of her opponents through the vibrations of the earth.?? Then she would react based on the movements.?? Think about when she had trouble with Aang in the tournament.?? When he is floating she couldn't sense him.?? Then later she even said that she feels with the ground.?? Like when she felt the ants in her backyard.
The Chase:
She blocked the one girls knives because she felt her wrist movement so she knew to throw up a rock wall to block them off.
Lake Laogai:
She sensed who was telling the truth based upon their heartbeat and breath.??
So if she can sense everything that easily then she can react easily.?? She has movements of earthbending memorized so she is able to react in a good defense.?? It is when she is overwhelmed that she can't react.
Now as for the other bending.?? None of them need to be in contact with their element to bend it.?? You will find out that for fire to work then you have to have the proper breathing.?? This about Tai Chi.?? In China it is believed that a human being can access their chi with the proper breathing and focus.?? So fire is an enrgy that is different from the other elements.?? It can be created by mixing energy with oxygen.?? That is why I am not surprised that Zuko has not made Aang's scooter blow up under him.?? It is air and in that air is oxygen that can feed any fire mixed with it.?? Waterbending is another example where they did not have contact with the element.?? Look at some of the times when Aang has bent the water.
