I know that the heroes can't die, or else the show would be over, but my point is that every time Azula loses, she loses in a very similar way: not because she underestimates her opponents (she doesn't) but because as soon as it is clear that she is winning, she gets cocky and lets her guard down. Over the course of the series, the writers/Bryke have been fond of giving their characters weaknesses and then having those weaknesses be the cause of their ultimate downfall (think of Zhao). If the writers hadn't wanted to develope this as a weakness, they would have varied the ways in which Azula is defeated (I first began to suspect this after noticing how similar the fights in The Chase and The Drill were).
As to your second point, I find it extremely difficult to imagine an all-powerful firebending master like Ozai admitting to anyone, especially Azula, that he tried to kill Zuko and not only failed to do so, but got thrown against a wall for his trouble. The whole reason Iroh originally taught Zuko how to redirect lightning was so that he could fight Azula, so I feel that when they do fight, it will probly be used.
I do agree with you however, that Azula is awsome (heck, she's my favourite character). Like any perfect character though, she has her flaws, which will ultimately lead to her downfall (one of the disadvantages of falling in love with the villain of any story).

It shows her reeling back and forth trying to keep her balance after the gondola jerked to a stop. By the time she regained her balance, Zuko had already pulled Sokka up.

She was flailing her arms around several times after the car stopped. By the time she realized what was going on, Sokka was already onboard and she was more concerned about amscraying than fighting at that point.
The other main reason why I think Azula being hit with her own lightning bolt is likely is because the writers are fond of having villains be defeated by their own attacks/actions. As well as the scene in 116 where Zhao accidently burns his own boats, Zhao was ultimately killed by the ocean spirit because he was the one who aroused it by killing the moon spirit. General Phong got most of his men injured and his fortress smashed apart because he forced Aang to go into the Avatar State. Long Feng allowed Azula to temporarily take command of the Dai Lee, and was soon betrayed by them. Hama was defeated with the waterbending technique that she had just taught to Katara. Combustion Man blew himself apart.
I think that pretty much covers every major villain who hasn't yet been dealt with, as well as some of the minor villains.
It's true, she does do that. But I don't think it's underestimating. In fact, she's incredibly genre savvy and is better at underestimating her enemies than a lot of villains you see. But she does take too long sometimes to attack; just to mess with them.
Azula is really on her high horse, and sooner or later, she's going to fall, and it's going to hurt.

She has a bad habit of counting her chicken-pigs before they hatch. Or, you know, whatever it is that chicken-pigs do.
